Instructions for Mail-in Registration

You can register for any class by mail. Classes will be filled on a first come, first served basis depending on postmark of each registration. Please do not sign up for a class without a committment to attend each week. There are no make-ups for missed classes. If you have any questions regarding the class your dog should be attending, please call the Dog Training Manager at (919) 304-2300 x 233.

1. Print and fill out the registration form completely.

2. Select a class, day and time. Be sure to list a second choice in the event that your first choice is already filled. ONLY THOSE WHO ARE NOT PLACED IN THEIR FIRST CHOICE CLASS WILL BE NOTIFIED BY PHONE! All other should attend their first choice class.

3. Read and sign the release statement on the registration form.

4. Enclose a copy of your dog's vaccination record. Each dog must have a vaccination record from your veterinarian including distemper, hepatitis, parvo, and parainfluenza (DHLP&P), bordetella, and rabies (for dogs over 4 months). This is for the protection of the entire class. If you do not have your dog's vaccination record handy, you must bring it to Orientation. Even if you have shown a vaccination record for a previous class, you must show a complete current vaccination record for this session. NO DOG WILL BE ALLOWED TO PARTICIPATE WITHOUT A CURRENT VACCINATION RECORD!

5. Enclose a check made payable to 'APS of Orange County'.

All classes will be held at the APS Education and Behavior Center at the Felicit Latane Animal Sanctuary.
